For new clients, please fill out the following form prior to your scheduled appointment.
When scheduling an appointment with our hospital, please be aware of our cancellation policy. We require at least 24 hours’ notice if you need to cancel or reschedule your appointment. If we are not given at least 24 hours’ notice, clients can incur a $50 per patient cancellation fee. If a client no-call, no-shows a scheduled appointment, this will incur a $50 per patient no show fee. If you need to cancel a surgical procedure, we require 24 hours’ notice. Should we not be given 24 hours’ notice, it will incur a $100 per patient cancellation fee. We understand that extenuation circumstances may occur where you need to suddenly cancel an appointment, and those will be considered on a case-by-case basis.
